We understand that changes to your dental practice can bring up questions, especially when you’ve built relationships with the dentists you’ve trusted over the years. Our goal is to make this transition as smooth and comfortable as possible.
Many of you have received care from Dr. Azar Mahmoudi and, more recently, Dr. Khan. When Dr. Khan made the difficult decision to step away from clinical practice due to personal circumstances, one of his biggest priorities was ensuring his patients would continue receiving quality dental care.
He took great care in finding the right team to take over the practice and ultimately entrusted Dr. Sadaf Moghimi, Dr. John Nakhla, and Dr. Mark Nakhla of Crow Canyon Dental with continuing that care.

What if Dr. Khan already recommended treatment?
No need to start from scratch. We have your treatment history on file and can review any previous recommendations with you, answer questions, and discuss the next steps.
Will you still accept my insurance?
We work with a variety of PPO insurance plans. If you’re unsure about your specific coverage, give our team a call. We’re happy to help verify your benefits before your appointment.
Do I need to transfer my dental records?
Not at all! Your dental records, X-rays, and treatment history are already securely on file, so there’s nothing you need to do.
